Low Ferritin Causes and What Your Labs Reveal

Fatigue that does not match your workload, slower recovery after training, reduced focus, and hair shedding are easy to dismiss as stress. But low ferritin causes deserve a closer look, especially when your bloodwork shows depleted iron stores before a standard blood count becomes clearly abnormal.

Ferritin is more than a number on a lab report. It reflects the iron your body has in reserve for making red blood cells, supporting oxygen delivery, producing energy, and maintaining healthy tissues. When those reserves fall, performance can suffer long before you feel sick enough to seek care.

What Low Ferritin Actually Means

Ferritin is a protein that stores iron. A low result generally points to depleted iron stores, often called iron deficiency. It does not automatically mean you have anemia, which is a reduction in healthy red blood cells or hemoglobin. You can have low ferritin with a normal hemoglobin level and still experience symptoms that affect work, training, sleep, and daily momentum.

The context matters. Ferritin can rise during inflammation, infection, liver disease, or other inflammatory states, which can make iron stores look better than they are. That is why ferritin should not be interpreted in isolation. A clinically useful evaluation may also include a complete blood count, serum iron, total iron-binding capacity or transferrin, transferrin saturation, and markers that help clarify inflammation.

The Most Common Low Ferritin Causes

Ongoing Blood Loss

For many adults, chronic blood loss is the most important cause to investigate. In menstruating women, heavy or prolonged periods can steadily drain iron stores month after month. This is common, but it should not be normalized simply because it is common. Needing to change protection very frequently, passing large clots, bleeding longer than expected, or feeling consistently depleted around your cycle warrants a medical conversation.

In men and postmenopausal women, low ferritin requires particular attention to possible bleeding from the gastrointestinal tract. Causes can include ulcers, gastritis, hemorrhoids, inflammatory bowel conditions, polyps, or, less commonly, cancer. Regular use of aspirin, ibuprofen, naproxen, and other anti-inflammatory medications can also contribute to gastrointestinal irritation or bleeding in some people.

Blood donation is another often-overlooked source of iron loss. Donating occasionally may be well tolerated, but frequent donations can reduce ferritin, particularly when dietary intake or absorption is not keeping pace. The same principle applies after surgery, childbirth, or any event involving significant blood loss.

Not Getting Enough Iron From Food

Dietary intake can contribute, although it is not always the whole explanation. Heme iron from meat, poultry, and seafood is generally absorbed more efficiently than non-heme iron from plant foods. People who eat vegetarian or vegan diets can absolutely meet their needs, but they need a more intentional nutrition strategy and may benefit from periodic lab monitoring.

Low intake is also more likely during restrictive dieting, appetite changes, highly processed eating patterns, or busy periods when meals become inconsistent. Athletes and high-performers sometimes unintentionally underfuel while pursuing body-composition or training goals. The result can be a mismatch between iron intake and the demands being placed on the body.

Poor Iron Absorption

You can eat iron-rich foods and still have low ferritin if your gut is not absorbing iron effectively. Celiac disease, inflammatory bowel disease, chronic gastritis, and prior gastrointestinal surgery can all interfere with absorption. Reduced stomach acid, including from long-term use of certain acid-suppressing medications, may also play a role for some patients.

Food timing matters too. Vitamin C can support non-heme iron absorption, while calcium, tea, coffee, and some high-fiber foods can reduce absorption when consumed at the same time as iron-rich meals or supplements. These interactions do not mean you need to avoid healthy foods. They simply illustrate why a one-size-fits-all supplement plan often misses the real issue.

Higher Physiological Demand

Iron requirements rise during pregnancy because the body is supporting increased blood volume and fetal development. Pregnancy-related low ferritin is common and should be managed with individualized medical guidance, not guesswork.

Adolescence, rapid growth, intense endurance training, and frequent high-volume exercise can also raise iron needs. Endurance athletes may lose iron through sweat, gastrointestinal stress, repeated foot-strike hemolysis, and exercise-related inflammation. That does not mean every tired runner needs iron. It means persistent symptoms and declining performance should prompt testing rather than assumptions.

Less Common Medical Drivers

Some causes are less obvious but still clinically relevant. Chronic kidney disease, autoimmune conditions, certain infections, and inflammatory disorders can alter iron handling in the body. In these cases, ferritin may be normal or elevated despite limited iron availability for red blood cell production. This is one reason low energy cannot be explained by a single lab value alone.

Genetic factors, medication effects, and combined nutrient deficiencies may also complicate the picture. Vitamin B12 and folate deficiencies, thyroid dysfunction, low vitamin D, poor sleep, and metabolic issues can overlap with iron-deficiency symptoms. A targeted workup helps separate a true iron issue from the other variables affecting your output.

Signs That Low Ferritin May Be Affecting Performance

The symptoms of depleted iron stores can be subtle at first. You may notice that your usual workout feels harder, your concentration drops in the afternoon, or you need more caffeine to function at the same level. Some people experience shortness of breath with exertion, headaches, dizziness, restless legs, brittle nails, hair shedding, or a racing heartbeat.

Symptoms are not specific enough to confirm the diagnosis. Stress, sleep disruption, hormonal changes, depression, thyroid disorders, and inadequate calories can produce a similar pattern. Bloodwork turns a vague complaint into measurable data and helps prevent the wrong intervention.

Why Self-Treating With Iron Can Backfire

Taking an iron supplement without understanding the cause may delay evaluation of an underlying problem, particularly ongoing blood loss. It can also cause nausea, constipation, stomach pain, or diarrhea. More is not necessarily better: excessive iron can be harmful, and not everyone with fatigue benefits from supplementation.

The right dose, formulation, and schedule depend on your results, symptoms, medical history, diet, and tolerance. Some patients do better with lower-dose or alternate-day dosing, while others need more direct treatment or further investigation into absorption and blood loss. The objective is not to chase a number. It is to restore adequate iron availability while identifying why stores dropped in the first place.

What a Smart Low-Ferritin Evaluation Looks Like

A high-quality evaluation begins with the full pattern, not just a ferritin flag on a lab portal. Your clinician should review your complete blood count, iron studies, medical history, medications, diet, exercise volume, menstrual history when relevant, and any gastrointestinal symptoms.

From there, the next step may be nutrition changes, medically guided supplementation, repeat testing, menstrual management, or additional evaluation for gastrointestinal blood loss or malabsorption. The plan should be specific enough to answer three questions: What is driving the deficiency? What will correct it? How will progress be measured?

Repeat labs matter because feeling better is valuable, but it does not always confirm that iron stores are fully restored. Monitoring helps confirm the plan is working, prevents unnecessary prolonged supplementation, and gives you a clearer baseline for future health decisions.

Low ferritin is not a productivity problem to push through. It is a signal that your body may not have the reserves needed to perform, recover, and stay resilient.
